• 8:30 am - 2:00pm

Please have the following with you at the time of registration:

• Student's Birth Certificate with State Seal

• Student's Social Security Card

• 2 Proofs of Residency (utility bills, tax tickets, lease agreement)

• Immunization Records (can be faxed to 304-755-3843)

• Dental Records (can be faxed to 304-755-3843)

• Health/Wellness Records if available (can be faxed to 304-755-3843)

• Copy of Current IEP/504 if applicable

• HS-18 Form (only if your child will require medication at school)
